It may be the socalled "contextual menu" hangs. This contextual menu comes up when you rightclick (ctl+click; doubletap trackpad). Quit that app where it happened and go back; or: "relaunch finder".
There is no control key or trackpad, nor finder on an ipad
try restarting the app, double tap the home button then swipe the icon of the app up and off the screen to close, relaunch and see if problem persists.

I have the same problem. The overlay covers the video in the small format and cannot be clicked/tapped. It behaves as though it was not there, i. e. I can click the enlarge button in the lower right corner. When in fullscreen mode the overlay is not visible anymore. It appears as far as I know in every video in Safari.
I have installed the latest firmware on my Ipad 2, closed Safari and restarted the whole Ipad. The overlay is still there.
I'm having this problem too. None of the proposed fixes work.
Try the reset:
Quit all apps that are open, at least Safari;
Hold the Power and Home buttons at the same time, keep holding, when you see the white apple logo let go of both. Let it start up.
As mentioned above: I have already done it ... several times.
Just now I logged in after a reset. I closed all tasks in Safari, closed the app, then double tapped the round button twice and threw all apps into heaven, then shut the power off, waited. And after opening safari and a new website with a video (<--Tap), what do I see? My friend the white panel again... :-(

The reset did not work for me either. Problem is still there.
I will try to sync the Ipad with Itunes on my PC hoping it will work. Till then...
FYI. I did also try to tweak my Java setting and Safari. Did nothing. The little white window still persists!
In an attempt to clarify this problem. I believe it only happens on videos played through Facebook. The problem will not replicate when I play videos directly through YouTube. Can anyone else confirm? If I'm right, it might not be us. Facebook may be to blame.
