unknown usb device (set address failure) Bootcamp 5.1

How do I repair this device drive failure on my windows 8.1 bootcamp installation? I assume it is my SD card reader given that it does not work in windows.


Side note: SD card reader works on OSx

MacBook Pro (Retina, 15-inch, Late 2013), Windows 8

After the firmware update discussed here boot - macOS Startup Manager is very slow and laggy

the issue returns to the sd card issue experienced with bootcamp before the windows 10 anniversary update - SD Card Slot Disappears After Sleep (Gone Until Reboot)

Hi there Loner T, thanks for replying.


What type of a Device is this? It's the SD card reader.

Can you check Event Viewer for any messages related to this device? Please guide me on how to do this? I had a look at the event viewer, but cannot find a way to filter the data to what I'm looking for.

What year/model is your Mac? Late 2014 MacBook Pro Retina 13"

Is it specific to the SD Card? Yes, the SD card isn't working. I re-installed Windows after my previous post, and the SD card was fine for a couple of hours, and the device manager showed no errors. Them it returned.

The only SD Card that I know if which works in a Mac running Windows is the one that comes with a Broadcom SCSI driver. Apple SD cards do not work consistently.


There is a very long discussion at SD Card Slot Disappears After Sleep (Gone Until Reboot) . There is a quite a bit of finger-pointing on whose problem is it. Windows or the Driver.


On W8.1, the way to look at events is, to click on the Events tab. Your signature says Windows 8, is that the version you current have?

I've had the SD card disappearing problem during sleep well, but always had it running again after rebooting. This no longer works.


This is what I get in the Events when re-installing the Apple SD card driver.


  • Driver Management has concluded the process to add Service AppleSDR for Device Instance ID USB\VID_0000&PID_0004\5&35EFF004&0&12 with the following status: 0.
  • Driver Management has concluded the process to add Service USBSTOR for Device Instance ID USB\VID_0000&PID_0004\5&35EFF004&0&12 with the following status: 0.
  • Driver Management concluded the process to install driver applesdr64.inf_amd64_276b49b06f08d562\applesdr64.inf for Device Instance ID USB\VID_0000&PID_0004\5&35EFF004&0&12 with the following status: 0x0.


The device status still states: This device cannot start. (Code 10) The USB set address request failed.


Bootcamp 6.0 supplies this as well as a BroadcomCardReader64.exe, but I cannot find this driver as an option when updating the SD card driver. I have Windows 10 Home installed.

For over three years now (see date of first post on first link below), a wide range of Macbook users have reported severe problems with the bootcamp SD card driver, we think for a particular Broadcom chipset. The issue got progressively worse with the release of Windows 10, and now with the Windows 10 Anniversary Edition (1607 release) the SD Card is inoperable: on boot we receive Set Address Failed (see last link for latest post) and no known workaround exists. Steps to reproduce:

Boot into bootcamp on a Macbook pro Retina (other users have found the problem in Macbook Airs also - see posts) which uses the affected broadcom chipset, running Windows 10 Anniversary Edition. Try to access content stored on an SD Card using the inbuilt SD Card reader.


Expected Results:

Device Manager (and an OS-desktop popup) will show "USB Set Address failed" as detailed in https://discussions.apple.com/thread/7352538?answerId=31154740022#31154740022 and included in the attached files

On a previous version of Windows you may instead suffer the SD card not resuming from sleep or some lesser issue


Actual Results:

Device manager shows "USB Set Address Failed" and no known workaround is applicable. On older versions of windows you may experience a lesser issue that still renders your SD card unreadable.

Note the number of users affected, the diversity of their hardware, the amount of time that has expired since reports came in and the perceived lack of faith from users in a resolution due to Apple and Microsoft (for whom users are both paid customers) not seeking to resolve the issue. Hence I would suggest this is very urgent.
