Ios 8 personal hotspot is gone!

i update my iphone 5s to ios8, but my Personal hotspot is missing from my iphone! What i can do, to get it back?

thanks .. I tried and the issue fixed with me using the above steps.
This worked. What i did was enter details in all the fields including APN, Username and Password (Any dummy details u can enter) and then magic.. personal hotspot just appeared !!!
