iOS 8 changed the way autofill works. Some sites will fill automatically. but others require that you tap in the text entry to bring up the keyboard. At the top of the keyboard there will be an option to fill the name and password. Tap on that. Then tap on Sign in or whatever button the site has for login.

Was having the same issue and finally licked it. The problem seems to arise when there are multiple versions of a username or domain saved in Keychain -- "Bob" and "bob," or example.com and www.example.com. The solution for me was to go into Settings on each iOS device and delete the saved user/password pair.

Settings > Safari > Passwords & Autofill > Saved Passwords

Click Edit, then check the user/password pair(s), then click Delete.

Recheck each device and make sure the user/PW pair has not repopulated in the Saved Passwords list. Then go back to the problem website and log in with your username & PW, saying yes to the Keychain prompt. Then log out and log in again. Put your cursor in the "Username" field. The keyboard should come up, with "Autofill password" just over the QW keys. If that doesn't work on an iPad, try the same thing on your iPhone, then go back to the iPad.

I've been struggling with this too and finally got it to work.

Settings/icloud/keychain

-turn keychain off but save the passwords on your device.

-hold down the power button until it powers down.

- Restart the device.

Settings/iCloud/keychain

-turn keychain on

-if you have multiple devices attached to the same iCloud account one of your other devices will have to allow this device, using the iTunes password.

I hope I've explained this properly because I tried so many times, I might be confused lol.

i Was having the same exact issue. This is what corrected it for me;

1. Got settings > iCloud>iCloud Keychain: Make sure it in on. Go to advanced: Make sure APPROVE WITH SECURITY CODE IS ON and the correct phone number and country is imputed.

2. GO to Safari> AutoFIll: Ensure all switches are in the ON position. This is a good time to impute credit card if applicable.

HOpe this helps. Let me know.
