The code *6112022115678# wouldn't have worked because you have to have a second star after *61

It should be *61*12022115678#

Actually I tried all *61*1234567890#, *62*1234567890#, *67*1234567890#, and **004*1234567890#. All get the error messages.

The individual CCF codes worked for me last week. But since I power cycled my phone, it went back to trying to use ATT voicemail, and now the *61* etc codes are not working.

Try using a +1 in front of the area code ie *61*+11234567890# this seems to get better results, I started using this method years ago with stored contacts on my SIM card that was causing a problem with Android phones. Also the method of using another phone to set the call forwarding may not work if the other phone is an android. ATT tells me that ATT and Android use different provisioning (servers) which can cause problems in and of itself as I've found out from switching a SIM card between phones when at work (Android) / home (Apple).

I just tried all the steps you went through too, and I am also having that problem, even with the manual changes with the *61 and the others. Hopefully this will be fixed by at least the next iOS iteration.
