Why is my Health Dashboard suddenly empty?

The Health Dashboard is suddenly empty on my iPhone 5s running iOS 8.0.2. What happened?

Same issue on my iPhone 6S iOS 9.3.4

The only fix I’ve found is a reboot of the iPhone.
