just noticed a similar thing with FB. i have 30 thousand photos, but when i go to upload a recent holiday photo it's saying i only have 5 thousand in my camera roll. and many photos are simply not showing up from my holiday just one week ago.
this is totally bizarre and frustrating...
I just discovered that to save photos from Instagram to the iPhoto camera roll the iCloud photo saving must be turned off. Go into iCloud on the home screen, then into photos and turn the 'save photos' off. It said I would loose all photos saved - this applies to the iCloud photo gallery and not to the device's iPhoto gallery.
Then I don't know, sorry.
Same here, but only on my iPad. It works fine on the iPhone.

Does anyone know if Apple/Instagram/Facebook plan on fixing this - I can see the use of icloud but if it makes instagram accessing photos difficult, then what is the point?

This solution worked for me as well. Do not store anything on I-Cloud. It confuses the **** out of Instagram;)

For me, it was because the images were "Live" images. I duplicated them as still images, and they showed up! 🙂
(Share button > Duplicate > Duplicate as still image)
