So, I setup a passcode on my Watch and I thought that it would stay unlocked while on my wrist and only require the passcode when taken off but that doesn't seem to be the case. Am I wrong?
I have Simple Passcode set, Unlock with iPhone turned on and also Wrist Detection turned on.
it is an intermittent fault, call apple ASAP before your warranty expires. Make a Genius Bar appointment and get them to examine, take screen shots or video of the locking too in case the fault disappears agaIn.
Definitely software related. My watch worked perfectly for many months, then this behavior started. I too asked myself what I had done to alter the watch in any way, and I remembered I had loaded the "Delta Airlines" app the day before this started. I removed the app from my phone and it disappeared from my watch as well. Everything is now working perfectly again. I unlock in the morning when I put it on and it is good for the day. Apple needs to diagnose what this app and possibly others are doing to the watch to make them constantly lock, and introduce a fix in the next update.
take it to A Genius Bar. They are aware of the issue, they have replaced both mine and my wife's .. Is a known sensor issue - it's very intermittent so you "think" you have fixed it, but it might return any second. Or not! It's a weird one certainly. I wouldn't let my warranty expire though and suffer through faults.

Just tried this and it worked.
1. Turn off passcode on Watch so Watch stays unlocked.
2.Then turn passcode back on again.
Seems to have fixed the issue for now. Will update if anything changes.
