Do you have any third-party NTFS software like Tuxera, Paragon, NTFS-3g (MacFuse/OSX Fuse) or it's variants? When did the Windows partition go missing? Was it after a Yosemite Recovery Upgrade?
Can you post the output of the following Terminal commands?
diskutil list
diskutil cs list
sudo gpt -vv -r show /dev/disk0
sudo fdisk /dev/disk0
The "sudo" commands will prompt for your password, and it will not be echoed back. You may also see a warning about improper use of "sudo" and potential data loss due to "abuse" of the command.

If Windows was installed using BC Assistant without using EFI boot, then you can use the following procedure. You will need to download GPT Fdisk from http://sourceforge.net/projects/gptfdisk/.
Rebuild MBR to match the new GPT information thus resetting the Hybrid MBR. Use defaults for other questions (like partition codes). The only values that need modifications are the Boot flags and step 10. Accept all other defaults that Gdisk offers.
Here is sample Q&A for this section. Please notice the Press Enter/Return.
Place EFI GPT (0xEE) partition first in MBR (good for GRUB)? (Y/N): Y
Creating entry for GPT partition #2 (MBR partition #2)
Enter an MBR hex code (default AF): Press Enter/Return
Set the bootable flag? (Y/N): N
Creating entry for GPT partition #3 (MBR partition #3)
Enter an MBR hex code (default AB): Press Enter/Return
Set the bootable flag? (Y/N): N
Creating entry for GPT partition #4 (MBR partition #4)
Enter an MBR hex code (default 07): Press Enter/Return
Set the bootable flag? (Y/N): Y
Test 1 - Does Bootcamp Volume show up in Finder?
Test 2 - Can you see files in Bootcamp Volume?
Test 3 - Can you select Bootcamp in System Preferences -> Startup Disk?
Test 4 - If Test 3 is successful, select Bootcamp and Click Restart.
Thank you Loner T for your detailed response . I'm off to work now , I will try everything you have told me and I'll report my findings sometime tomorrow . Thanks once again .
I developed a problem trying to follow the above. I am running OS X 10.10.5, on a Mac Mini. I have Tuxera NTFS installed. I was able to dual boot into either Windows 10 or OS X, view Bootcamp files via Finder, but Bootcamp did not show up in my System Preferences Startup Disk. I can no longer boot into Windows 10. I do have a Winclone backup. Can you offer some help?

It may be a bit easier to keep context, if you start a new thread. Post these two outputs that you have here , and also post the output of the following two commands
sudo gpt -vv -r show /dev/disk0
sudo fdisk /dev/disk0

What year/model is your Mac? Do you have any third-party NTFS software?
macbook pro 13 inch retina late 2013
8GB - 2.4GHz - core i5 processor
I have installed Paragon NTFS but I have been following Apple bootcamp official support instructions.
And today installed win7pro 64 bit successfully.
Windows doesnt detect usb or sound
but mouse and keyboard working fine so read official apple support and they told to goto startup disk select bootcamp
but mine doesnt show bootcamp
Please see https://kb.paragon-software.com/article/204 as a reference. System Preferences will not shown any NTFS disks, controlled by third-party NTFS drivers.
yeah I disabled paragon from settings pan then its showing windows partition ...
but new problem arises...
after selecting windows partition under system preference --> rebooted mac
went in to mac but no Boot Camp service icon neither in control panel nor anywhere else !!
do you know how to get rid of it ?

Sorry, but you selected OS X -> System Preferences -> Startup Disk -> Bootcamp and you booted into Windows, correct? If yes, you are in Windows, and do not have a Bootcamp icon in Windows Control Panel, correct?
What do you want to get rid off?
yes I did that way and it suppose to show my USB disk where I have saved windows support software so I can go in and open setup.exe to install window drivers . This is the procedure explained on apple bootcamp support forum.
Boot Camp missing from Startup Disk
