Currently, I have about 10 copies of each "default playlists" like the Top 25 Most Played playlist is duplicated 10 times and it's all screwed up. It doesn't show my most played songs, it shows 25 of my most recently played songs. I keep deleting the duplicates and they keep coming back.
Not the case with me, I never delete playlists from my mobile device. It's when I add a song or songs to a playlist on my Mac, that it auto-populates, with a 1 on the end of it.
Your suggestion SirMehdi didn't work... in fact, I have more than 1 playlist that has been copied now.
Make sure you delete these duplicated playlists from your itunes playlist section
Been doing the same thing for the last 12-18 months, wish Apple would patch this error.

Im kinda glad Its not just me having this issue but also disappointed that there has been no patch for this.

I've found when i make changes to playlists on my iPhone it creates the duplicates. Only solution I've done is to delete the playlists from my phone then sync them from scratch.
For Windows see my script KillTopLevelPlaylists. Put the playlists you want to keep inside playlist folders first.
