iOS 10 notification using flash

I keep my phone on silent during the work day and use "LED flash for alerts" accessibility setting.

Under iOS 9 this worked well, I would see the flash and when I looked at my phone there was a notification I had configured to give me an alert or a sound - an important email or text message, etc.

Under iOS 10 I am getting a lot of "ghost" flashes - the flash goes off but there are no notifications listed when I look at the phone. I've been going through, looking for notifications to turn off but I am still getting this.

Is there some way to get a definitely list or view of what is causing these ghost notifications?
