This finally solved my problem too. Turning up Bluetooth volume is done the same way as turning up the phone volume (side buttons on the iPhone) but has to be done while a call is in progress - ie while the Bluetooth is engaged! You will see it say Bluetooth volume on the phone screen when you do this. Turn the volume down on the car console (doesn’t have to be super low, just maybe below 40) and turn the Bluetooth volume up on the phone itself. This did finally solve my echo problem.
The issue is not with Apple, it is with Toyota.
Did not help. Still horrible echo. Can these two big companies not figure it out?

I have had my Highlander for almost two years. My Bluetooth was working fine then about 2 months ago people started complaining about the echo.
Where is the bluetooth volume. I have searched all over the settings in my phone but find nothing.
I have the same issue with my 2015 Toyota highlander- it started about 3 months ago- - went to Toyota- they updated the software on Highlander. Unpaired and re-paired IOS device and Highlander Bluetooth-updated IOS-reset network settings- still have issue--turning down sound in car helps somewhat but still echo.
My wife's iPhone 8 has this same problem with her 2018 Highlander Limited. I think I finally discovered a fix. On your iPhone, go to Settings, Accessibility, and turn off Phone Noise Cancellation. When I did this, the phone worked fine, no echo. I only wish I would have discovered this sooner. Before this, I took it to the dealer and got a code sheet to try, none of the codes fixed the problem. We have been living with this terrible echo on the receiving end of phone calls since we bought the car in November 2017.
I have a 2010 Toyota Highlander and an iPhone 8. I started to have the same problem with the callers voice echoing on their side. I took my car to the dealer last week and they fixed it. I had to call someone . While we were on the phone the mechanic had me turn up my Bluetooth volume all the way up on my phone. It didn’t say volume it said Bluetooth on my phone screen when I pressed the volume up button. Now I don’t have any more problems using my Bluetooth. He said the Bluetooth should remember the settings now.
This has not worked for me. It is still not usable.
