login issue

Could use some help from the community with a login issue.


iMac 27 Late 2015

Core i7 8 GB

OS X 10.11.6


The iMac does not necessarily display all five user accounts on the login screen.

Earlier today 2017 02 24 the iMac would not respond to the correct password in the unlock screen box.

Tried various work arounds.

Was able to log out and log in on secondary admin account.

Checked primary admin account in System Preferences - Accounts and Users.

All in order.

Logged out.

Primary admin account not displayed on login screen.

Logged in to secondary admin account. System Preferences - Accounts and Users.

Selected display user name and password box.

Shutdown / restart.

System did not display user name and password box.

Did not display primary admin account.

Logged in to secondary admin account. Logged out.

System did display user name and password box.

Logged in to primary admin account.

Experienced multiple requests for keychain password. This is a change.

System Preferences - Accounts and Users. Changed user password.

Keychain Manager. Synced user password with Keychain password.

Ran a backup. No issues.

Shutdown / startup.

Back to same problem, icons not boxes, and two accounts not displayed.

Reset NVRAM and SMC. No change.


iMac is usable by way of secondary admin account but awkward.


Not sure how best to proceed.

Revisited keychain account passwords. All five user accounts appeared to have matching user and keychain passwords. I reset the password on the principal admin account and keychain to new matching passwords. Did not appear to make a difference.


The content of the keychains is of course different for each account. Event here 2017 02 24 is one year plus two days past startup day for this machine. That does suggest that a key may possibly have expired. However, in keychain access the keys present today do not show expiration dates.


EtreCheck appears to indicate no adware or malware detected.


EtreCheck does report keychain Access hang on 2017 02 24. That's the hang that occurred while looking for work arounds about one hour after the system first failed to recognize the password for the unlock box. That hang was stuck in wheel for about one half hour before force quit and shutdown.

In System Preferences / Users&Groups/Login Option this change does not take effect.


The next shutdown / startup does not change to name and password, and does not show the icon for the primary account. What shows is three of five accounts on this machine, counting Guest as an account. Two do not show.


The workaround of logging in to secondary admin account, then logging out, does show name and password. Thus I still can get in to primary admin account.


Primary never shows in icon view. This is a change.

You are welcome. Long shot.


Might be a corrupt .plist.


Do a backup, preferably 2 backups on 2 separate drives.


Quit the application.


Go to Finder and select your user/home folder. With that Finder window as the front window, either select Finder/View/Show View options or go command - J. When the View options opens, check ’Show Library Folder’. That should make your user library folder visible in your user/home folder. Select Library. Then go to Preferences/com.apple.systempreferences.plist. Move the .plist to your desktop.


Restart the computer, open the application, and test. If it works okay, delete the plist from the desktop.


If the application is the same, return the .plist to where you got it from, overwriting the newer one.

Update.


Reset NVRAM curiously generates two startup tones, not one, both of them weak not the usual sound level.


Machine will not restart in safe mode (shift key). Goes directly to login screen.


Apple Diagnostics ran and reported no issues found Reference Code ADP000.


With that result, Apple Support assigned an Apple Case ID and called. They agreed to help work this issue.

Apple Support is still working on this.


Meanwhile set up a bootable external drive starting with new media.

No problem erasing the media and setting Mac OS Extended (Journaled).

GUID partition table.

Installed OS X 11.6 from Recovery Mode startup. No issues.

Restored from TimeMachine backup accepting all the categories of data.

Launched and executed ok.

TimeMachine backup launched immediately from this startup.

I chose to cancel that action as I wanted to think about the effects.

Restart.

Login screen is restored to normal - icons for all accounts - or name and password box as selected.

Runs correctly from restart. Selects external drive for startup.

No issues to see. Based on a very quick look everything appears to be present and working.


At least a partial success.


Based on this result appears next step is to call Apple Support, and then re-install OS to internal Mac HD, preserving user data.

One more question:


From the external boot drive startup, Finder prompts for passwords for the both the external Time Machine drive and the internal Mac HD.

Finder accepts the password for the external TimeMachine drive.

Finder did not accept the password to unlock the internal Mac HD.

What password is Finder prompting for?

Main admin account p/w

Keychain password

Master password

Apple ID password

Finder does prompt for password, not recovery key.

I was a little surprised, as I had understood "Master password" to be valid in all contexts on this machine.


Throughout this exercise Apple ID password is unchanged.

Update.


Apple Support is no longer working on this.

iMac worked fine on external bootable drive, re-installed OS X and Time Machine restore.

Went in to re-install OS X 10.11.6 from recovery drive.

Process in recovery mode did not recognize primary admin account password.

Did not recognize master password.

Process did recognize secondary admin account password.

Re-installed OS X on internal Mac HD ran fine but only briefly.

No change in password issues.


Started up again in recovery mode.

Disk Utility.

Erase process has failed.

Unable to delete the core storage logical volume.


Restart to recovery mode.

Disk utility.

Selected First Aid.

First Aid could not be completed.

Cocoa error -1.


iMac now will not complete startup under secondary admin account.

Startup makes progress partway.

Then black screen rolls down from the top, displaying only a white circle with a bar.

Shut down with power switch.

2 TB Fusion Drive shown.

1.3 TB used, 700GB available.

Fusion Drive in this case meaning OEM dual 250 GB SSD and disk combination.

Mac HD not shown.


iMac continues to start up and operate correctly using external drive.

More about iMac:


iMac 27 Late 2015 purchased refurbished directly from Apple February 2016.

Thus some element of data structure on disk may possibly reflect previous owner.

Initial problem noted 24 February 2017, one year after first startup this location.

Thank you Eric Root and others. The iMac 27 Late 2015 is once again running correctly. The trip to the Apple Store proved instructive. The technician used Apple diagnostic software to check the machine for hardware problems, finding none.


Then the technician looked at low level logical structure of the FusionDrive, describing it as a complex system, in which software causes one solid-state drive and one disk drive to emulate a single disk drive. The low-level software developed some kind of problem, such that the disk utility program did not recognize a Macintosh HD partition on the FusionDrive.


The tech indicated he had seen this issue before with FusionDrive. The tech also stated that this had nothing to do with the setup of this particular machine or use of FileVault. The issue is with FusionDrive.


The answer was to clear both the SSD and disk drive using unix commands in Terminal, and then to create the correct logical structure, starting from the beginning. These are the steps taken, approximately, for illustration only. The tech worked from notes he had taken on a previous occasion when he worked this problem.


diskutil zerodisk disk0 < caution: command syntax incorrect - illustration only

diskutil create Mac HD


Once these steps were completed correctly the tech installed a clean image of MacOS Sierra 10.12.3. The operating system setup ran normally, taking a restore from the most recent backup volume.
