For both your cases, if you have a 2011 Mac, UEFI is not properly supported. You will need to install Windows 7/8.1 using the built-in Optical drive (not a USB installer) and then upgrade to W10 in-place, since there are no Apple-certified W10 drivers for your specific model.

Can you post the value of BIOS Mode?

What is the year/model of your Mac (do not post the serial number)? Are you connecting the TB-to-HDMI before you boot Windows or after it has fully booted?

Run msinfo32 in Windows and post the value of BIOS Mode.

Your BCA should have only two options, Download and Install, and Install looks for a DVD in the Optical drive.
