I recently updated to High Sierra 10.13.4 and ever since, the fan is running on full speed, all the time. According to the Activity Monitor, the CPU is used less than 15% and the machine is not running hot. The fan stops briefly when I turn on Siri, but starts again after a few seconds.
Anyone else?

wow-i just had this happen on a less than 1 month old MacBook Air and searched for help! It sounded like it was going to take off. After following the link below from John Galt, I opened activity monitor and it was my running Pages that was causing the increased CPU usage. I had never used the program before and may not use it again after this. Mac went back to normal as soon as I quit out of pages.
I too had this problem after installing 10.13.4 update. It was driving me nuts. Finally, after reading many comments, I went to Activity Monitor and found that my Winclone app Backup Menu Item (option in the menu bar) was sucking up my CPU usage. I turned off the menu item, and the fans are now silent. Hope this helps.
I´m able to correlate that Finder is becoming unresponsive ~1min into any command while Safe boot works fine. It is definitely a GPU issue. I´m running MBPro mid 2015 with full power, clean install 10.13.4 and the only way to interact with the system is to use Path Finder to force Finder to quit at login. Every app and pref pane that requires interaction with the builtin finders browser becomes unresponsive for ~3-5 mins, sometimes completely. It helps to disconnect externals USB drives, but as my work largely depends on external storage, macOS becomes quite useless.
TLDR = Try Path Finder and tell it to quit finder and PF creates it´s on desktop. I´m currently running PF8 (beta) and it´s very fast, but some minor glitches and update from day to day. It crushes Finders performance though and it seems like my laptop is allergic to anything Finder related right now.
- oh yeah... My fans are completely bipolar. I can literally track the sound of a software iteration loop and thus far, the only solution has been to boot into safe mode, followed by normal. Safe boot works very well and is sort of what Im resorting to in the meantime. The fans are either SUPER SPINNING or quiet.

I have MBP15 2017 maxed out and am having the same issue. When plugged into Ultrafine 5K, CPU usage only 9-10% but fans are running at full speed after OS update.

I have the same problem with an iMac after update 10.13.4.
Shutdown and SMC reset did not help.
CPU usage about 9-10% only but fan still runs.
10.13.4 was a very big update. How much RAM do you have. 4Gb would be barely sufficient for High Sierra after the update, if not before the update.
16GB. It’s definitely not memory pressure.
I have 12 GB RAM, I do not think that is the problem.
I have tried SMC reset. Didn't work.
Also try turning off Spotlight. That worked for my MacBook Air.
