It's hard to say exactly what it is without a whole lot more troubleshooting. However, compose your selfie then look in the correct direction.
If you have time, make an appointment at the Genius Bar at your local Apple Store to have them check everything out.

So look toward the diagonally opposite corner of the screen.
That does get your eyes aimed in the right direction but is still very awkward as you can’t look directly at the screen when composing shot so don’t know exactly what you’re going to get. Hope Apple fixes this annoying bug.
I’m using the front and centre but tried everything and eyes are always looking at corner of screen no matter what I do. Might be a glitch on this iPad. My old mini iPad never did this.
I don't think it's a bug in Apple's software. If it were, there would most likely be more people complaining about it.

Have you tried to take a selfie without holding the iPad in your hands?
You could set it upright on a shelf or table with some support in the back and sit comfortably in front of it. Adjust all seeing to your liking, then release the shutter with the timer. This will give you ten seconds to look directly into the camera.
Use the capture timer to give yourself time to be in the shot. First set up iPad where you want, then frame your shot. Tap the timer button, tap 3s or 10s, then tap the Shutter button.
I'm frequently using the timer when I want to avoid a sudden motion while releasing the shutter.
Are you using the front or back camera? I would recommend using the front. Ensure you’re looking at the center of your camera, and try changing the angle you hold your iPad at.
You're welcome. I just suggested it as test, if it possible at all to take selfies with your iPad while looking straight into the camera.
