Help troubleshooting kernel panics & startup problem

Would anyone be able to help me troubleshoot my iMac? I am a busy school teacher that has a large family and the nearest Apple store is the better part of an hour away. I would love to avoid spending 3-5 hours overall hauling this up there and maybe having to leave it there if I can possibly do so.


Lately I have had the following behaviors on my 2011 21.5" iMac:

  • kernel panics where the machine freezes (sometimes spinning beach ball, others non-spinning beach ball, other times an unprompted reboot followed by a message that the machine was restarted because of a problem). I am not able to detect a particular program causing this.
  • yesterday I found the machine with a folder with a question mark on a grey screen as though it could not find the start up disk. I used the power button to force it off and start up again and it functioned fine.

I am going to post 2 panic logs in the comment as they appear different (at least in length).


Things I have tried:

  • My time machine drive is no longer recognized by the machine. I believe this is unrelated (as it does not work on another machine either), but it stopped backing things up 2 weeks ago. I have unplugged it and still had kernel panics since then.
  • I am just now unplugging my USB hub with phone chargers and 2nd external hard drive to reduce peripherals connected. Not sure if that is going to help yet.
  • I tried rebooting into recovery mode to run a disk repair on my internal OS X running SSD (I put that in a few years ago alongside the HDD), but disk utility reported an error I cannot recall now saying it could not complete a repair. It did not identify the problem from what I can recall.

Try booting into safe mode Use safe mode to isolate issues with your Mac - Apple Support allow it some time as it will do some file system repairs, the reboot normally.

Also please run and post an Etrecheck report so we can get an idea what is running on your Mac http://etrecheck.com/

High cpu usage can be triggered by a lot of things, Etrecheck could help identify the reason.

So far I have not had another kernel panic, as far as I can tell, (have also disconnected external hard drives), but I continue to have programs quitting on me randomly. For example, yesterday the Finder restarted for no obvious reason and iTunes quit unexpectedly in the middle of playing a song, and then this morning I woke to a crash report for firefox and in the activity monitor it has been showing Dropbox is not responding for the last hour and a half (force quitting that now). So, it does not look as though I am done on this.


I notice 31% of my CPU is being used by kernel_task with a CPU time of 46 minutes and 148 threads being shown in activity monitor. Is that normal?

I did reboot into safe mode and other than web pages paging (I think that is what it is called when images redraw rather than scroll smoothly), things worked as expected while I had it booted into safe mode.


Here is an Etrecheck report for restarting (I am not sure if the check is the same for beach balling or software crashing, which I have also been experiencing).

I think we may be on to something with this hardware test. It found 1 error:

4MOT/4/40000003: HDD-1241


What seems odd about this is that I do not have my system running off the HDD. I installed an SSD and have the system running off of that, but have my user account located on my HDD.


Another possible piece to this puzzle is that when I went back to try and run a repair disk on the SSD I got an error while checking the fsroot tree

error: drec_key object(oid 0x6b6c9): invalid has(0, expected 1d15c) of name (.store.db)


Any clues as to whether these errors indicate failure of either drive?

Note that to the Mac, your SSD is a HDD, so don't be confused by the terminology, it could be your SSD that's bad.


In particular, if Disk Utility found damage to your SSD, it may be a flaky SSD.


However a "4MOT" failure indicates a fan motor may have died, and your iMac may instead be overheating, causing issues; it's quite surprising how quickly heat builds up on the motherboard when a fan dies and the seemingly random errors it can cause. I can't say for sure as Apple doesn't publish the Hardware Test codes, but I would lean towards the suspect fan being the one dedicated to cooling the hard drive.


If you installed the SSD yourself, it could be a matter of a cable having been displaced when you were doing the drive swap, a cable being routed improperly or a cable not replaced properly, impacting cooling and/or fan speed detection, resulting in the error.


If you did it yourself, it might be worth your time to disassemble it again and check.


If not, I would take it in for further analysis, as if it is the fan, you'll have nothing but issues until it's replaced and working properly.


The other fans in the system will try and compensate, but there's only so much they can do.


Even if you swapped the SSD in yourself, you may still want to find a repair depot, as the procedure to replace a HDD fan is decidedly lengthy and complex (according to iFixit there are 46 steps and it will take one to two hours), and after all that there is no guarantee that the fan itself is what is causing your problems.

History: I did the SSD install in 2013. The HDD I swapped last year for a larger one when the original one showed signs of failing.


Isn't there a utility people use for monitoring fans and temperatures? Seems like I had one at one point.


I would try the fan replacement if I can find one to buy. iFixit is out of the part. I have this particular machine apart 4-5 times and done other iMacs and iPhones without trouble and am up for the challenge as this machine is probably not worth $300 and the depot price I am guessing would be at least $150. Maybe I will check with my local computer shop that claims they work on Macs.

So, since my fans seems to be working for the HDD and it is possible the SSD is what was really indicated as failing, maybe I will just try and install the OS on the HDD not use the SSD for a while. If problems go away, that seems to be the logical cause.


My only doubt would be if the diagnostic really were referring to the HDD fan - that would not be explained by the SSD failure.

I assume you installed th SSD in the optical drive bay? If the SSD were the problem I’d suspect that AHT would have indicated a problem with the optical drive not the HDD. I think 2011 iMacs were among the group that used circuitry onboard the hard drive, instead of external temp sensors, to control the HDD fan, 99% of the times I’ve seen a 4MOT HDD code it has pointed to a failing hard drive.

I still have my optical drive. This model actually sold with an optional SSD that Apple would fit behind the optical drive. Here is a guide I followed when installing: Installing iMac Intel 21.5" EMC 2428 Dual Drive Kit - iFixit


Mike - would it still point to the HDD failing if it is not used for the system - just the user home folder?


Maybe I should create a new user and run off of that for a while and have it all on the SSD so the HDD is not accessed for anything? Let's call that option 1.
Alternatively, I could follow my previous plan and install the OS on the HDD and not use the SSD for anything. Let's call that option 2.


Which do you think is the more logical next step?

AHT don't check for software, system or otherwise, it checks hardware only. I looked at the iFixIt guide after I posted and saw the dual hard drive how to, but am wondering how does the system sense temperature of the SSD? Unless I missed it, and I could have, I didn't see any mention or installation of a temp sensor.

I partitioned my HDD thinking I would install the OS on the new partition without disturbing my data on the HDD and the partitioning went fine, but the install failed twice stating there was a hardware problem and to run the hardware diagnostic we ran earlier. I ran it again just to be sure and had the same error. Not sure if that is indicative of anything in particular as perhaps part of the install process simply runs a similar test and halts the install process before moving ahead.


I think I should try creating a new user account that is only on the SSD and see if there are issues then. Will try tomorrow.
