I have a sheet I’m trying to add some forms too, see picture bellow. But when ever click the + button it just creates a new sheet, no option to add a form, does anyone know if this function is still available and how to do it or what I am doing wrong,

Yes, I just verified I can still do this on iOS (iPad). Remember, your table must have header columns turned on in the table settings, not just formatted to look like headers. If you don't name the table, it will be assigned a sequential numbering like Table 1, then table 2, etc...
I also made a table of just check boxes to make sure that wasn't an issue, and it worked just fine.

Sorted, my issue was i was located on my iCloud, once moved to iPhone it worked fine
When you say header columns I presume you mean this
do you mean the file was on icloud, or you were using numbers on icloud?
Is the add form function on Number still available
