Imac slow and beachballing

A few days ago my iMac started lagging. I got an EtreCheck but i`m to bad at computers so i can`t understand what to do. Could someone with a little more experience help me out? This is the report:

You probably will, neither Disk Utility or SMART is very precise at predicting when a disk will fail. The only other thing that could be slowing your disk is Little Snitch but it isn't running the your report. Until you replace the disk, keep good backups.

If you want to test first, install an OS on a clean external hard drive and test that to see if you it is still beach balling. That way you'll know it is the internal causing the problem.
