iPhone 7 Plus: Calls - No Audio

Question

When I make or receive phone calls, I can't hear ANY audio in the call.

The problem exists for Phone calls, Whatsapp calls, Facetime calls...

I have not tested other apps.

The only (temporary) fix i have found is a phone reboot. I am rebooting my phone pretty much every time i need to make a call (of any kind) now. The problem reappears less than an hour after each reboot.
If i hit the Speaker button, it lights up and then turns off.
You can also try to route the audio in a call through a bluetooth headset/speaker, this works somewhat unreliably. But if you select the internal phone or speaker option all audio disappears.
All the other apps that need to use the speaker (apple music) work fine!
I have reset my phone to factory defaults, I have restored iOS, i have restored from iCloud backup. The problem is persistent and repeatable.

How do I fix this problem?

It doesn't look like it's a hardware issue or the audio would always be broken.

It looks predominantly like it is a software bug in the Phone app, but what is causing it?

I am running the latest firmware: iOS 11.4
