Imac Late 2013 High CPU usage, fan goes on and on, tried reset SMC and PRAM nothing changed

Description of the problem on an iMac 21,5 late 2013 (Mac OS 10.13.5 and downgraded to Yosemite 10.10.5): from a month to now I noticed the high speed fan and my activities in the imac, already very slow, practically paralyzed. used the High Sierra and had upgraded to 10.13.5. Searching, I used the activity monitor and noticed a high CPU usage by the Kernell Task. I researched the communities and followed the following steps, many times, with a temporary solution or no solution:

1) SMC Reset (various)

2) Reset SMC and NVRAM (several)

3) Restart in safe mode - problem continues

4) Rodo the Apple Diagnostics - finds no problems

5) Restart in recovery mode, check disk - all ok

6) Restart in recovery mode, reinstall the system from the internet - no solution

7) Restart in recovery mode, restore the system to before the update via time machine - did not work

8) Restart in recovery mode, restore the system to the first day of use of the original time machine, high sierra - did not work

9) Restore the system: clean install of Yosemite (last system I had stable in my imac late 2013) - no solution

10) Restart in recovery mode, repair the disk permissions - super right, success for 24 hours, and so it went on high cpu usage again, everytime I get into recovery mode and ask to repair permissions. It works for a while (more precisely until the hd sleeps, when the system returns the high usage of the cpu continues)

11) What has changed between the High Sierra and Yosemite: in the high sierra the cpu does not fall from 60-99% of use. In Yosemite, with an empty hdisk since I have not refilled all my files (from 600GB of occupation to 100GB), it is high so, but when it sleeps the fan turns off and when it returns, it is still possible to work for 5 minutes before there is overload the cpu. Yosemite also does not stop the machine, despite the overload I can do more common tasks like seeing / editing photos, movies and texts. In the high sierra I could not do anything else.

12) I forgot to say that I used a hair dryer to try to blow inside the imac without opening it, if it was some kind of dirt on the fan or temperature sensor (yes, I read all the posts in several mac communities). If anyone has another suggestion other than to take to the technical support, thank you.

Uninstall Dr Cleaner and Dr Anti-Virus as per the developer's instructions. No Anti-Virus software or so-called “cleaning” apps are needed or recommended for Mac OS. They can conflict with Mac's own built-in security. At best they will slow your Mac by using unnecessary resources and at worst will bork your entire system.

After that do a restart and post another Etrecheck report.

Done, no results. Repairing disk permissions again have stopped the problem for a while, but it has returned. Below I'll post another result by EtreCheck, and I noticed that time machine is not being able to finish a backup, something is freezing it. I'm going to an apple store today in order to describe all this to a technician, I'm almost convinced the problem is hardware, both physical or logical. Anyway, do you think it's a good idea to clean install high sierra? Thanks for your help.
