There's an ever present black box ~1/2" x 1/2" with a white sold circle inside that follows all screens and apps. Once touched, it opens to a larger box containing shortcuts to: Notifications, Custom, Gestures, Home, Control Center and Device. How can I stop it from appearing?

Go to settings down to Safari delete history and data.
Then on iPhone try a reboot press the power button down &menu button hold both down until you see apple logo.
