On the App Store, I notice some apps have the cloud icon where the ‘Get’ or buy button usually is, and this is for apps that I downloaded and deleted years ago. Does that mean they’re still on the cloud and taking up storage? If so, how do I remove them for good?

No, it does not mean that. Purchases that you make in the iTunes and App Stores can be stored on your device, or stored in iTunes in the Cloud (which is not the same as your iCloud account). It simply means that the app is a previously purchased app and it is still connected to the Apple ID that you have signed in, which allows you to simply re-download it if you want to at some point. It has no effect on your iCloud storage.
