In iTunes, you can. But when using Spotify from my Macbook, I'm only able to choose one output at a time - one HomePod, for example, but not both. Using Spotify on my iPhone 7, I don't have this problem; I can choose multiple speakers. What gives?

Unfortunately not, AirPlay 2 is not currently able to output at a system level within macOS.

Even better, I just learned that next week's Mojave release will include the Home app, allowing not just iOS devices but also OSX devices to select multiple speakers at the same time using AirPlay2 (currently enabled in iTunes on MacBooks, but not in third-party apps like Spotify). So this problem will go away within a week.
Well, that's disappointing. We may buy an iPad when the new ones come out and use that for music; I presume any iOS device works like my iPhone does (allowing multiple audio outputs via AirPlay 2)?
All iOS devices capable of running iOS12 have the ability to support the AirPlay2 Protocol. Any new iPad in the works should definitely support.
