Spam callers bypassing Do Not Disturb

Three times in the last month I have been awakened in the middle of the night by spam callers dialing twice in a row and bypassing Do Not Disturb. I keep my phone on because my mother is ill and I have medical power of attorney for her. Her caregivers need to be able to reach me in an emergency no matter what time of day. Does anyone know how to keep this from happening? If not, could Apple possibly develop the ability for users to block all callers during Do Not Disturb mode except phone numbers specifically allowed by the user?

Check Settings>Do Not Disturb>Repeated Calls and set it to "Off".

That is already a feature of Do Not Disturb. Settings>Do Not Disturb>Allow Calls From. Set it to Favorites and add the caregivers to Favorites. Or, set the caregiver's ringtones to Emergency Bypass. Open the contact for the caregiver, tap Edit in the upper right corner, Tap ringtone. Turn Emerbency Bypass to "on". Tap "Done" in the upper right corner.
