I'm using the latest iPhone with the latest OS. I have motion and fitness activated. However when I put face down it doesn't dim the display or silence notifications. Has this feature been removed?

try this:
disable Facedown Detectio by access the “Fitness Tracking” toggle and disabling it via the following menu. This also works the other way around for if you notice that Facedown Detection is not working properly. See screenshot. Turn off and on. Then restart phone.
This is the only link I can find that is discussing this. I have an XS running 12.1 and it does not work. Now the fun fact. My wife's iPhone 7 and daughter's iPhone 8 have stopped working. All phones running ios 12.1
What version of ios are you running?
