How to remove feedback app?

What feedback app? IF you restore your phone as new, then only the default apps will be on your phone.

You were running the beta and that is the Feedback Assistant for it. Neither you nor we are allowed to discuss anything beta related, so you will need to contact Apple. Once installed, I believe FA remains even if you are on a publicly available iOS version unless you unsubscribe from the public beta.

Slightly hold down on that app until it wiggles and delete it.

Or go to settings - general - iphone storage - scroll down to the app click on it and delete app.
