My headphones are plugged into my iPhone 7, but will not recognize that they are plugged. Hence, my music will play outloud. I believed it was the headphones at first but they work perfectly fine with my iPad mini. I have dusted off the headphones, factory reset my iPhone and restarted it multiple times.

Use a wooden or plastic toothpick and see if you might have a bit of pocket lint stuck in the headphone jack. I also find that a quick blast of canned compressed air is helpful in blowing out the fluff or lint. Hope that helps.
