iTunes just starts up by itself and it plays 1st song on my list

i'm having a problem that seems to be driving me crazy. itunes seems to randomly start up and play the first song in my library. just to note:

- i am running no 3rd party itunes widget
- only add-on i have is last.fm
- i have tried permission repair

does anyone know what is causing this. i really won't stop. i quit itunes, 10-15mins later its open again. any thoughts on what could cause this/how to stop it?

24" imac, Mac OS X (10.6.1)

Thank you... this was very helpful. My iTunes kept turning on and when I unplugged my headphones, the problem resolved.

I plug and unplug the speakers, iTunes starts playing. It's a reproducible trigger. Your solution seemed to handled that problem.
