Hey guys, I live in Brazil and am using an iPhone 4 bought in China which is presenting the same symptoms described in this post (other people can't hear me on a phone conversation, regardless of me using the internal mic, speaker, headphone or bluetooth earpiece, but otherwise other apps work just fine). I tried the "solutions" mentioned here but nothing worked and it really seems to be a software issue. The worst part is that, despite my iPhone being in the 1-year limited warranty, the fact that it was bought in China makes the warranty useless in Brazil according to Apple.
Based on the number of hits on this thread it seems like a realy unfair move from Apple not to address this issue more directly, it's clear we're not talking about a couple hundreds of malfunctioning iPhones. I'm definitely not going to buy a new one.

So i was experencing the same problem as most described. If there was an incoming call and i did not pick it up after the first ring it would hangup on the caller. If i picked up the phone the caller could not hear me and i could not hear them. I called an apple tech and they walked me through the usual restore procedure and made me restore as a new phone versus from a backup. Still had the problem. Which led them to believe that it was a hardware issue.
they sent me a temporary iphone to use while they worked on my phone. when i got home i plugged it in set it up and the same problem persist. which led me to conclude that it was either my itunes or an app conflict. So i erased all the content on the phone and continued to setup the phone while connected to wifi and through icloud. Once all the settings were restored and the phone powered on i prompted a friend to call me. amazingly i could hear them. (icloud was still slowly working on uploading all my apps) once all the apps were downloaded onto the phone i once again could not hear my incoming callers. this brought me to the conclusion that is an app conflict. i have not figured out which one. So i again erased the whole iphone and set it up through wifi on icloud. right when the phone powered on i went to settings>icloud>storage & backup> and then chose the option to stop the backup. I tried the phone and it worked.

I had this intermittent issue where callers could either not hear me or hear static. I did not have the iCloud backup turned on. I did a complete reload of the software and reset the network settings on the phone but the problem always came back. I finally got to the point where the problem occurred all the time so I decided to open it up. I removed the battery and the speaker/mic assembly to check for debris but I found nothing. I also could not find any liquid damage on the board. I reassembled the device and now the phone works great. The only thing I believe it could've been was that the screws may have loosened and the board was not grounded. I've seen this happen to laptops just from typical transport or movement. Just something to consider if you see this problem however if your phone is still under warranty, have Apple service it.
Within 30 days of having my iPhone 4s White 16 GB, I noticed this problem of other people not being able to hear me so I took my iPhone into an Apple store. They were able to confirm the problem of having issues with the mic (after taking it to the back to test it out) and replaced my iPhone. However, it was a refurbished iPhone since when I asked if it was new, the Apple representative said that the interior parts of the phone may have been refurbished. She did say refurbished iPhones go through more extensive quality check for defects. Did you, or anyone on here ever receive a brand new iPhone, or was your replacement a reburished unit?
Any opinions on whether I "deserve" a brand new phone would also be appreciated (I mean I paid for a brand new phone and basically got a lemon as it was a hardware issue of the mic, then I "settled" with a refurbished phone).
If you were truly within 30 days, you should have received a brand new phone. If your carrier is AT&T they would have given you a new one.
I agree with the above posted. Also Apple need to address this MIC issue as now it showing up on the new 4gs too! I also have the same issue and went to apple regarding it. Since mine is out of warranty period, I have to paid 150 buck to get a new one ( replacement phone). These replacement phone come in a black case and it will be use to ship the defective phone back to apple. With all these posting about MIC issue, apple really need to address it and hopefully will soon or bye bye apple.

I have been having a similar problem, and I believe this is a fix for anyone who has similar issues.
First, this requires you to open the back cover. I recommend an exchange if you can.
My problem is this:
1.All apps can use the mic without problems.
2. Only during calls does my mic not work, whether bluetooth, speaker, or regular.
The assumption I had was a network problem. I've read about sim card issues, and thought about it and found this youtube video

What I did was just open up the case like the guy. I pushed down around the area where the sim card is, and I can make a normal call again. I just did what he said, folded a piece of paper 4 times and let it sit in the space where the sim card is. I didn't do an L shape. Just that rectangle area that seems to have a gap. Replaced the cover and everything is good again.
It gives me the assumption that it's a faulty sim connecter. I think apple should address this. Again, this fix is probably just for those who have the same issues as me.

I'm having the same problem with a 4S. Its intermittent and I can't figure out any pattern. Sometimes happens when Bluetooth is off, sometimes when it is on. I can always hear the other party, but sometimes they can't hear me.
I've been reading these posts and it seems the only way to solve the problem is to get it replaced. What's annoying to me is that this 'new' phone was a replacement phone from apple because my previous iPhone 4 wasn't making sound when I got an incoming call or text, or even when I typed. No, my phone was not on silent...it seems like something internally messed up when I dropped my phone. Now I have the replacement and no one can hear my on the recieving end of the line. I think I'd rather have my old messed up phone and couple missed calls/texts versus this useless new thing. Ugh. Guess I have to make a trip back to the apple store tomorrow. Crossing my fingers the new 'new' phone works correctly!

It seems to have worked.
On my girlfriend's 32gb iPhone 4G, after doing a restore from backup to latest firmware, this issue persisted without syncing any new apps to the phone. I didn't put in her iCloud password, so it wasn't restoring her apps from there, either. I opened the multitasking bar and the only things open were phone, messaging, clock, and camera. The only one that would use mic would be camera (for videos) so I closed that. After closing, made a call and it worked fine. Sounds like an App conflict to me.
It would seem that the issue is something to do with the hardware and or software related to the noise cancelling microphone (and or the IC that is used for audio compling) - which is not used it every app. Although I am not sure why that affects incoming audio.
I have an appt tonight at the local AppleStore for my iPhone 4 - which is a few months past warranty coverage.
Tell us how it goes, mine is out of warranty as well... would like to see them do something about this for you without paying out of your pocket.
About 6 months out of warranty for me.
After the usual questions about restore etc. they decided it must be a hardware fault.
Also. Aside from battery replacement, Apple considers iOS devices to be non-servicable items. Meaning they do not send them out for component level diagnosis or repair.
Leaving the only AppleStore option is a hardware replacement at $150 plus tax for my 16GB iPhone 4.
The only other official option is to deal with it on your own. An option which could include soldering the audio IC or finding another unit with a different problem and swapping parts. Or since the iPod stuff still worked on mine I could have demoted it to an iPod and got a new phone but that would mean tripping the contract etc.
I decided to do the $150 swap out in part because the glass was chipped at bottom front so they may have been other damage.
I suspense I could have tried the resoldering and then still did the swap out if it did not work. But that would have meant a couple more days with no phone and more effort etc. and though I have done component level repair of capacitors and other items, everything inside the iPhone is so tiny I wasn't sure I had the proper tools or skill necessary to pull it off.

There's a lot of nonsense being suggested here.
This "no-audio-output-when-calling" fault is becoming a more common fault on the iphone-4 - due to the fact that the IC (chip) repsonsible is poorly soldered to the motherboard (dry joints).
If you dropped your phone, that reveals the bad joints (damages them further)
Usually the motherboard can be repaired by replacing the IC
