I think the snapshot tells a lot.
After years i wanted to change my profile picture.
Done once, worked as expected. (I picked one from my photo library)
Tried a second time and this is the result: no menu, just a blank flag.
Any idea about this?

Suddenly, solved.
I did again what dialabrain suggested some posts ago,
I dragged and dropped directly onto the previous picture a new one.
This time a tiny one (around hundred pixels square png). This makes a difference? I do not know, anyway, it worked...
Done. Unlocked, back to normal.
Now whatever is the picture I try to use it works like it should.
Even if the reason of the locked menu is still not clear,
the solution was finally what dialabrain suggested.

One does not need to click the lock icon to change the picture. I am running Mojave 10.14.3 (18D109) and when I click the circular image, I receive a fully functional sheet with controls to select another image. I have Automatic login set to Off, and Display Login windows as List of Users.
No system preference would be deleted,. Only changed to the default assuming you changed any from the default.
