Gently tap and hold on an app until they wiggle then drag it to where you want it. Swipe up from the bottom when done.

Off of the Dock? Touch and hold an app lightly until all of the icons start to wiggle and then slide the app off of the dock -- or if you want to delete the app from the phone, tap the x on the upper left corner of the icon (if it is an app than can be deleted).
