Can only watch a movie in one language

The distributor/rights holder determines the content. If they chose not to make the English audio available in your region, there's nothing to be done about it.

Does anybody know, is there a workaround to purchasing and watching movies in English, short of using VPN or setting a country code in iTunes settings to a different country, which doesn't seem very convenient?

It would also be a violation of Apple's Terms of Sale and could lead to your account being suspended.
