My elderly aunt who is in early dementia stages has an iPhone 7 which is a lifeline for her to stay in touch with friends and relatives; however, during her mental lapses she plays with the phone and often deletes icons and I have to go in and restore, sometimes with difficulty.
Is there some way to lock the icons so that they aren't deleted?

No. But what you might consider doing is this.
Use Guided Access with iPhone, iPad, and iPod touch - Apple Support
See if this helps: go to settings, screen time, content and privacy restrictions, iTunes and App Store purchases, deleting apps, don’t allow.
