Comments or notes on pictures in camera roll
how can I save a comment or a note on my pictures or videos on my iphone camera roll (saved pictures)?

You need a third-party app to annotate your photos with titles or captions as embedded metadata tags, like for example Photo Inspector, ViewEXIF, or Metapho. You may want to have a look at the available photo editing extensions at the AppStore. Usually you need the full, paid versions to be able to modify exif tags.
