Cannot setup ECG on Series 4 Watch

I have apple watch series 4 but cannot setup ecg. The “setup” option does not appear in my Health app / Heart / ECG.

iOS and WatchOS are all upgraded. Based in UK.

if it was purchased in a country where ECG is not allowed it would be the cause
