I used disk utility and formatted the drive after booting with CMD+OPT+R.
After the second attempt to download & install MacOS onto iMac this error came up. Although I’ve done dozens of installs this has never been an issue. So far, I’ve found little documentation on this type of error.
Is the macOS installer also seeing one or two volumes?

If installer is seeing two, then you need to erase the drive again until the installer only sees one and install macOS Catalina on that one.
What year model iMac?

What format did you use on the drive?
(later macOS's require GUID/APFS)

Erase the disk, as per > How to erase a disk for Mac - Apple Support
Then reinstall macOS, as per > How to reinstall macOS from macOS Recovery - Apple Support
Yes it shows the 2 volumes but to the right under the 1TB logo it says “shared by 5 volumes”.
I have erased all drives and now all I can get is a white screen with a question mark on a file icon.
Now I can’t get back to recovery nor will it boot to safe SHIFT while booting.

I'll agree, that life was simpler before 10.13.

Erased drives prior but in the top right it says the 1TB is shared by 5 volumes.
Also ran 1st Aid on both drives and everything checked out ok.
Catalina should run on the device, I checked the official list.
