Watching Apple TV Plus on MD199Ll/A

Each time we try to watch Apple TV Plus we are being asked to log in (again) with Apple ID.

When password is provided a verification code is then sent. However there is no screen/place to then enter the verification code.

how do we get this resolved so we can enjoy the service.
