Unable to access Apple TC+ after logging in

My Apple TV (it's an older model, but it still supports the Apple TV+ app) asks for my login credentials (after a complete factory reset and software update) which seem to work. When I open the Apple TV+ app, it offers the free one-week trial, but clearly is not aware that I subscribe already. When I try to enter credentials for the one-week trial, it fails. It seems to be stuck in a loop where it asks for Apple ID credentials over and over, but never sees that I am an Apple TV+ customer.

When on an older Apple TV and signing in with 2 factor authentication you need to enter the password and code together. In order to stay within the time limit you need to enter the password, then manually request the code, and add it to the password
