Does this occur for a different user, e.g., Guest? How to test an issue in another user account on your Mac.
Does this occur after starting up in Safe Mode?

If you don't have another user account create another admin user account, Set up users, guests, and groups on Mac, and see if the problem persists. (This tells us if the problem is limited to your user account or is system wide). The account will take up very little space on your boot drive but is an excellent troubleshooting tool.
The symptom you describe is consistent with startup disk failure.
