Hi Amynthys.
Thanks for following up and letting us know that the issue is not that the sound is not heard.
You mentioned that the volume is changing on your iPhone. If you're using an iPhone X or later, please check to see if 'Attention Aware Features' is enabled. This setting allows the TrueDepth camera to check to see if you're looking at the iPhone, and will then lower the volume of alerts: About Attention Aware features on your iPhone X or iPad Pro - Apple Support.
If there is no change, or if that feature is turned off, go to Settings > Sounds & Haptics. Do you have 'Change with Buttons' turned on or off? Does it make a difference if it's on or off, or does the same behavior remain?
To be sure that we've covered all the basics, have you also tried restarting your iPhone with these steps: Restart your iPhone
If the issue persists let us know which apps you're seeing this issue with.
Also, what is the iOS version number showing when you go to Settings > General > About > Software Version?

I don’t connect to a car. In fact, one of my devices doesn’t use audio at all.
I don’t connect to a car either. Actually my Bluetooth is completely off since someone advised that may be the issue, but it’s not.
My screen time has never been on.

I just checked my girlfriend’s XR. It’s connected to her car and at full volume. Same 3.5.1 iOS as I run on my 7. If I change phones because of this it won’t be an Apple product.
I'm trying this as well as I also have a GMC with infotainment system and I did use the car yesterday and sure enough volume this morning was so low i missed my alarm.

I have never turned “screen time” on! Still have the problem.
I never had this issue until updating to iOS 14! Mine will turn off volume randomly whether Bluetooth is on or not. They have to fix this!
My case history is growing. If Apple can’t come up with a fix, they might agree to a replacement to see if it is a hardware issue. I’m not implying they will but I’m also not ruling it out.
Yep sake issue through numerous iOS updates. Now running 14.0.1...
How hard can this be to just fix? Deaf eats the purpose having a phone if you can’t hear calls doesn’t it...
Since iOS 12 🤔.... “sounds like” really doesn’t give much hope. A simple bug with huge issues. It should have been addressed a LONG time ago ...

how did you reset? There are two options in the menu "Reset":
Did you
"Reset all Settings"
or
"Erase all Content and Settings"
