Using family set up how do I add an email account under the mail program. It just says None when I go to settings and Mail program.

To clarify, I've set up the watch for a family member that is under our family sharing group using the family setup feature that was introduced in watchOS 7.Set up a family member’s Apple Watch - Apple Support. From this process I don't see how to add the mail account. I'm not sure it is possible.
How do you set up a mail account?
