Unknown photo keeps appearing

I tried searching about this in case someone had already asked and also on the web and didn’t seem to find any answers. Today i noticed while looking through my photos that there was a random black background photo with the words “What can i help you with?” on it and a weird bluish blob looking thing underneath. It had a time stamp on it of when i was at work, nowhere near my phone. Again tonight after deleting the first one another appeared. I thought at first it was “Siri”, but Siri responds to you, why would Siri leave a random photo on my photo album. Anyways, no idea where it’s coming from or whats making it do that. Freaked out that something or someone has access to my photos made me deny access to photos from all my Apps until i know how this is happening. I don't have iCloud syncing, nor do i have any “shared” albums”. My phone security also doesn't show any breach.

It may be a screenshot of Siri. You may want to check your Settings for Siri. If you have Siri allowed to come up, while the iPhone is locked, Siri may be triggered, when the iPhone is moved and could come up, even if you did not handle the iPhone yourself. Or an accidental pressing of the side buttons might take a screenshot of Siri on your screen.
