iPhone app keeps on reappearing even after DELETING it a billion times !

You can also try deleting it from the settings. Go to Settings>General>iPhone Storage. Let your list of apps load. Swipe to find the Instagram App and then tap on it. Delete it in the next screen. Then restart the phone and see if it is gone.

If Settings, Screentime is on then temporarily turn screen time off and try again.
