I'm at 14.4.2 and fails to update to the next version. I get the same error each time: The iPhone 'my iphone name' could not be updated. An unknown error occurred (4000). Followed by an OK and More Info button. When I click More info it suggests I restart both devices (comp and iphone) and retry, which I do but no change.
It all starts when I get this in iTunes:

I tried the one thing I didn't initially try, which was to connect directly to my computer, and not through the usb hub. That did not work.
It may be helpful to know that I used to have an 6 and now I have a 12
Try this - (it worked for me)
On your phone - switch off the passcode feature. Now connect to the PC and run the update process. It should now complete OK. Lastly - re-enable the passcode on the phone
Try updating over the air wirelessly and not through your computer. It works.
Well, I turned off the passcode feature and the update proceeded.
An hour later it's stuck at like 2%. Any thoughts?
