I’ve recently started using my Apple Watch swimming in open water, when I have finished my swim the watch doesn’t give me accurate readings at all.
The distance is out and the time is way out?
any ideas ?

I suggest trying the good old calibration steps first:
Get the most accurate measurements using your Apple Watch - Apple Support
The next corrective action would be to unpair and re-pair the watch with your iPhone. That will return the watch back to factory level settings, while keeping your apps and data intact.
If that is still not helping, it would be best to contact support with this issue.

That doesn't sound right. Have you included some free style swimming strikes in that workout, raising your arms clearly above the water level. That should help getting some accurate map coverage.
And did you select Open Water Swim as the workout ?
Here is a sample I recorded on S3 last summer. The dotted lines are breast stroke.

I swim with my mate who uses a garmin 40/50 swim. My watch tells me I’ve only completed a 18min swim and 500m

yes I have included everything but still the watch won’t record my swim accurately.
