While your phone may say the software is up to date, check the bottom of the page where the update to 15 is available. In details about the update you will see it allows you to download and store vaccine and lab results in Health app. Worked for me, hope it does for you.

The QR code might not be supported. It looks like only SMART Health Cards are supported according to the link in the Apple support page.

Add verifiable COVID-19 vaccination information to Apple Wallet and Health:

Is your provider that administered the vaccine one of those listed in the link below?

After struggling with this for a bit with the same issue where the apple.com page kept opening (on iOS 15.2, Safari, "Share to Wallet" not presenting, using Verifiable SMART Card QR code) I solved the issue by clearing my safari browser settings.

I went to Settings --> Safari --> Clear History and Website Data

After doing that, I was able scan the QR code and this time it immediately worked as expected, importing into Health App and Wallet.

There are a couple of ways to work around this issue. From previous posts:

If you’ve upgraded to iOS 15.1 and scanning the QR code opens the Apple Health website in Safari instead of the Health app itself, click on the share button in Safari. A menu should show an option to open or add to the Health app after clicking the share button.

Follow the prompts to add to Health and Wallet.

OR

  1. Open the page in Safari
  2. Long click on the "add to Apple Wallet & Health" link/image
  3. Select "Open in Health"

I was having this problem as well and I was already on iOS 15.1.1. No amount of restarting or force closing things seemed to help. I use Chrome as my default but I also tried copy/paste into Safari directly and still no luck.

What ended up doing the trick for me was going in to my Chrome app settings (in the Settings app itself), and set my default browser back to Safari. Once that was set I was able to tap on the link from the California vaccine records site and things worked great.

I don’t know if this will help the folks who need to scan a QR code from another screen but hopefully it’ll help for some of the crowd in here.

I have the same thing happen when I scan the QR code for my vaccine record. I do not get the share with Health App option, just the apple.com/health webpage. Cannot even find the permissions for the camera to make it share with the Health app. I have even tried the "Share" option with the downloaded file of my vaccine record to add that way into the Health app. There is not option to "Share" with Health in all the choices to use.
