While your phone may say the software is up to date, check the bottom of the page where the update to 15 is available. In details about the update you will see it allows you to download and store vaccine and lab results in Health app. Worked for me, hope it does for you.

Solution - for the rest of you folks from Saskatchewan, my solution was to download the SK Vax wallet app from the Apple App Store. Follow the steps, but be ready to scan a copy of your QR code. It will also give you the option to save to your iphone or iPad. Then when your QR code is required at a business, you can just open the app. Good luck y”all.
Agree with the previous resolution comments, the key is iOS 15.0.2 update to get your camera to recognize QR code to add to Apple Health app. Not yet possible to add to Apple Wallet. But you’ll either have to print QR code or take photo of QR code on another device to have your own camera recognize it to add to Apple Health app.

I had the same problem. But I did read that this was a feature on IOS 15.0.2 onwards. I did the update and it the QR scan went to the Apple Health app and added my vaccine verification.
Same exact issue happening to me!
Install IOS update 15.0.2 then it will work.
I have done the IOS update and have the covid health information but the QR code isn't in the health app
