iPhone 6s unresponsive screen

My phone goes to the "Enter Passcode" screen, and I am stuck there as the screen is unresponsive to touch. Hit the power button and it will sleep, hit it again and it will bring up the locked screen (time, date, notifications, etc). Press home to unlock, and it's back to the passcode screen. Hold the power button to "power off" and there is not response by the slider or Cancel button. Force power off, and it reboots but the above situation does not change.

Any help greatly appreciated

Troubleshooting an Unresponsive Screen:

A. Hold It Right There! - Avoid a Lockout: Enter the passcode incorrectly too many times, and you'd likely be locked out for good. So, hold it right there, and refer to this link: If you Forgot the Passcode on your iPhone, or your iPhone is Disabled - Apple Support. It would be the one you've always entered. If you do not know it, then look for it in a location you may have stored it in.
