The reason it doesn't work is because the QR code contains personal information, and thus is written to only be properly read by a Smart Health Card-compatible reader, which is only available starting in iOS 15. Once you upgrade your device, it should work for you.

After IOS 15 update....I receive this popup when trying to upload....this worked on 2 of my phones but not on one other that I have.

Updated the ios to 15. It does not work.

Did you update to 15.1?

Then I would try a forced restart of this device and then a restore from backup. If neither works click on the Get Support link at the top right of this page and/or contact CVS.
